me + my medicines is a new communications charter which aims to improve the way we talk about medicines. The charter will be launched at a free event on Thursday 12th October in Leeds, and patients, carers and health professionals are invited to attend.
The charter is a project which has been developed in collaboration with Leeds CCGs and patient and carer participation groups to bring about improvements in patient relationships with medicines and receiving the right help, at the right time, and from the right place.
The aim of me + my medicines to support and encourage good conversations between health professionals and patients so they can collaboratively resolve issues about the use of medicines. This will benefit patients, professionals and the NHS by improving medication-taking, reducing costs of medicines and waste, improving patient confidence in managing medicines and conditions, and a reduction in unnecessary GP appointments and hospital admissions.
This is a project which I have contributed my experiences as a patient with multiple long term conditions, including talking in a video with others about our experiences of communication around medicines which will be shown at the launch. I will be attending the launch and I hope to see you there!
